ID Proof

• Original passport (Preferred) or Original Aadhar Card
• Copy of passport (1st and last pages) or copy of Aadhar (if Presenting Original Aadhar Card)
• Original Birth Certificate of babies with a copy, if a passport or Aadhar card is not made
  2 recent photos
• Note: The details have to be filled in caps.
• Full Name, Residential Address, Contact Number, and Personal Email Address on the back of the passport copy of each applicant, to avoid delays

Other documents as per type of visa

• Work Visa: Company ID card and its copy or copy of the offer letter or sponsorship letter.
• Student Visa: Copy of the offer letter from the college/university.
• UPFRONT Medical Exams cannot be done for the following visa types: PR Visa and their dependants, Express entry visa, and those who have submitted their Visa application to IRCC.
• For the above visa types, please bring the Medical Examination Instructions Letter and the IME Form (IMM1017E) from IRCC.
• Please bring a hard copy (Xerox) of your COVID-19 Vaccination Certificate with ALL dates of your vaccines, if available.
• Below 18 years, a copy of immunization (Childhood vaccine) record details printout if available.
• Above 18 years, a copy of immunization (Childhood vaccine) record details and printout if available.
• Adult vaccines are available at the centre.
• Please print and bring the Medical History Sheet duly filled.
• Note: It is mandatory to bring with you all old and recent medical reports, prescriptions (including for eyes), X-rays, with reports and certificates. Especially if you have a history of TB or cancer.
• Reports will be submitted on the IRCC portal via eMedical.
• You will receive a soft copy of your reports within 10 days.

Rs 6400/- per applicant (above 15 years) (Blood test, urine test, chest X-ray, ophthalmic, and physical check-up)

Rs 4000/- (11-115 years) (Urine test, chest X-ray, ophthalmic, and physical check-up)

Rs 3200/- (5-11 yrs) (Urine test, ophthalmic, and physical check-up)

Rs 3000/- (below 5 years) (Physical check-up)

Charges will be extra for additional test requirements.

• You will receive an acknowledgement letter (information sheet) within 2 hours after the medical exam (self collection) with payment receipt.
• Fasting is not required.
• Only candidates are allowed in the OPD. However, if the candidate is under 18, one of the parents must accompany them. If the candidate is over 75 years old, then one relative is allowed.
• Female clients who are pregnant and wish to proceed with a chest X-ray will be required to sign a consent form, and the X-ray will be done with a protective double lead shield.
• Female clients who are pregnant and do not wish to proceed with an X-ray chest will be issued a deferral letter, and the case will go on hold.

However, if they do not wish to do an X-ray during pregnancy, the X-ray exam can be set aside and may be completed after delivery.

0 to 11 years: Rs 2000/- per applicant for general examination (no X-ray required).

• Fasting is not required.
• Female clients who are pregnant and wish to proceed with an X-ray chest will be required to sign a consent form, and the X-ray will be done with a protective double lead shield.
• Female clients who are pregnant and DO NOT WISH to do a chest X-ray can opt for a sputum exam for MTb.
  The whole procedure may take 2-3 hours, including issuing the certificate.
• A certificate will be issued to the candidate only (self-collection for above 18 years).
• Please bring your payment receipt to collect the certificate.
